TRANSITION TEACHER/SPECIALIST

Reports to: Building Administrator (DOHS)

PURPOSE STATEMENT

This position aims to teach two classes a day, manage the transition program, provide on-site supervision for students on the job site, and case manage/coordinate services for students with disabilities. 

 

JOB SUMMARY

The Transition Teacher/Specialist will perform various duties providing appropriate services to special education students preparing for life after graduation. This is accomplished through direct support, consultation, student and staff training, and technical assistance with accessing resources. 

 

ESSENTIAL FUNCTIONS

  1. Create, coordinate, facilitate, manage, and track all aspects of assigned students IEP’s.
  2. Linkage to the community to ensure successful post-school outcomes.
  3. Assist with case management building/school wide
  4. Assist general education teachers with modifications/adaptations of district curriculum.
  5. Work with inclusionary practices within the buildings.
  6. Distribute copies of any updated IEP documents (IEP at a glance) to IEP team members.
  7. Create a system of collecting and sharing ongoing progress monitoring data.
  8. Facilitate/write behavior intervention plans when needed.
  9. Meet all due dates requirements for IEP timelines.
  10. Provide community outreach education for recruiting and retaining employment services for our students.
  11. Act as the district transition contact person for administrators, teacher, parents and community partners.
  12. Provide consultation to special education staff regarding transition opportunities for students.
  13. Assist the Director of Special Education to develop, implement, and monitor a district wide plan to provide appropriate and effective transition services. 
  14. Work collaboratively with agency representatives to provide information sessions for students and parents, conduct eligibility and application activities
  15. Fulfill other duties as assigned by the Special Education Director or building administrator.

 

SKILLS AND ABILITIES

  1. Ability to effectively address a wide range of academic and social needs including at-risk, cultural diversity, English language learners, and special education students.
  2. Basic understanding of transitional law, transition service requirements, and implementation strategies
  3. Ability to communicate effectively with staff, administrators, students, teachers, parents, and community partners
  4. Ability to organize and supervise
  5. Ability to work collaboratively with multi-disciplinary teams.
  6. Ability to work with multiple agencies in the community.
  7. General understanding of work-based learning
  8. Ability to interact and collaborate positively with parents, students, and community members.
  9. Ability to develop and monitor contracts for work based learning sites in the community.
  10. Ability to communicate effectively with parents and work sites to organize and schedule students work site assignments and transportation. 
  11. Ability to record and collect data to analyze student performance on work based learning sites.
  12. Ability to create data driven decisions regarding instruction, reteaching, support and site based assignments for students job related behaviors.
  13. Ability to maintain confidentiality. 
  14. Ability and commitment to engage in relevant professional development in order to strengthen job-related competencies. 

MINIMUM QUALIFICATION :

 

EDUCATION: Bachelor’s degree with appropriate endorsement, Valid Washington State Teaching Certificate, Master’s degree, or Doctorate from an accredited university or college.

EXPERIENCE: Appropriate academic background and coursework for the position. Successful teaching experience and effective work with high school age students, preferred. Previous experience in work-based learning and/or Career Education (preferred). Implementing transition requirements in the secondary educational setting (preferred).

CERTIFICATES AND LICENSES: Valid Washington State Teaching Certificate with appropriate endorsements. Valid WA State driver’s license and appropriate insurance required. Must maintain as current any certification or license listed herein and held at hire date.

CONTINUING EDUCATION/TRAINING: That which is required to be effective in the position and to maintain certification

CLEARANCES: Criminal Justice fingerprint/Background Clearance

FLSA STATUS: Exempt

 

PHYSICAL DEMANDS

  1. Physically able to occasionally lift and/or move up to twenty-five (25) pounds.
  2. Physical ability to walk, stand, stoop, bend, crawl, lift, reach, pull and climb as
    required to satisfactory perform required duties.
  3. Ability to focus (adjust eye to bring an object into sharp focus), clear vision at twenty(20) inches or less, and clear vision at twenty (20) feet or more.
  4. Must be able to work safely in varying hot and cold (weather) environments.
  5. Frequent standing, walking, lifting and bending.

 

CONDITIONS OF EMPLOYMENT
Starting Date: 2024-2025 School Year (180 total contracted days)
Salary: Based on education and experience Per OEA CBA - 2024-2025 Salary Schedule 
Retirement: Pension through Washington State Department of Retirement Systems
